Desperate for team building ideas. HELP! PLEASE!?

Primarily female staffed medical facility, and no one has been there for more than 18 months. Catty, competitive and territorial behavior and the patients are catching on. Unfortunately the patients have to be there for several hours of treatment and I NEED ideas for a company outing or event that might bring us all together or give us the opportunity to get to know one another.
For one thing, try to come up with something to do during work hours. Initially, employees resent being asked to do something during their time off.
You can make it literally team work by dividing up the staff and have some form of friendly competition with a tangible incentive for the winners. I don’t know what to say specifically, because you must have some sort of customer satisfaction measurement, or percentages of work results you would have to make specific for your office team. Good luck! I know this is a challenging area.
How about finding one of those rope and climbing exercises? It builds trust, confidence and support among a team. I heard it works quite well.
